Age Concern Hampshire are committed to promoting and supporting the independence of people over 50 and offer a wide range of much needed services to vulnerable elderly people in our community.
We have 6 Care and Wellbeing Centres across the county which specialise in supporting people who require extra assistance due to their physical or mental health needs. We provide a caring and safe environment where everyone can make friends and have fun. Our fantastic staff are also trained in person-centred dementia care and support so carers can have the peace of mind that their loved ones are in safe hands.
We also offer:-
• A Hospital to Home service, designed to improve the patients’ experience of being in hospital and support their journey through rehabilitation and discharge;
• A Help at Home service which offers friendly and reliable help around the home;
• Foot Care clinics;
• An Information Hub which provides impartial and confidential support
